For further inquiry or to make an appointment

Please call the Relate Center or send an email and one of our clinical professionals will contact you. The focus of the initial phone call will be to discuss your needs and familiarize you with the Center. We will then schedule an initial consultation.

The Treatment Process

Treatment will begin with an assessment. Given the Relate Center’s comprehensive approach, there may also be further medical or physical therapy assessments See Our Approach for further information. After the assessment, a tailored collaborative treatment plan will be created. There will be periodic reviews of the plan to assess progress in meeting your goals.

Payment

Payment for your initial visit must be paid at the time of your appointment. You will be provided a payment statement that you may then submit to your insurance company and/or any other third-party payers. We cannot guarantee reimbursement. The Center is not responsible for the collection of such payments. Payment methods include check, cash and credit card.

Cancellation Policy

Should you need to cancel an appointment, Relate requires cancellation prior to 24 hour advance notice to avoid paying for the appointment.
